Terms and conditions for Unlimited Monthly Membership

Welcome to FIT4MOM Phoenix Scottsdale offering Stroller Strides, Stroller Barre, Fit4Baby, Run Club, Strides 360, Body Ignite and Body Well classes to the Phoenix Scottsdale Area! Below you will find our policies and information on how to contact us with any questions or concerns you may have regarding your membership or class pass.

MANAGING YOUR MEMBER & CLASS PASS CLIENT ACCOUNTS

All FIT4MOM Phoenix Scottsdale Members and Class Pass clients may manage their personal accounts by visiting: https://phoenix-scottsdale.fit4mom.com/ and selecting the “My Account” tab along the menu bar. This will direct all clients to our Pike13 client account management system. Clients may also access this site by visiting https://fit4mom-phoenix-scottsdale.pike13.com/accounts/sign_in directly. All FIT4MOM Phoenix Scottsdale Members and Class Pass clients must have their Liability Release waiver signed online via their Pike13 account prior to participating in any of our programs.

PAYMENTS FOR CLASSES

We accept payment by debit cards, Visa, Discover, AMEX, or MasterCard credit cards or cash. All monthly memberships must have a credit card on file. Payments must be received in full prior to beginning any class. Billing periods will reflect a 30 day cycle beginning on the 1st day of purchased services. Monthly payments for debit/credit cards will be charged automatically every 30 days for all memberships. If a Member should change debit/credit card companies, have an updated expiration date, or a new billing address, these changes must be submitted via client Pike13 accounts. Funds that are denied for any reason will be subject to a $15 re-processing fee. Please see below for our monthly hold and cancellation policy.

CLASS PASS PAYMENTS

We accept debit cards, Visa, Discover, AMEX, or MasterCard credit cards for passes. Cash will only be accepted for drop ins or class pass purchases- Payments must be received in full prior to beginning any class

STAY INFORMED

Our main method of communication with you regarding FIT4MOM Phoenix Scottsdale programming is through e-mail. If you do not receive our monthly newsletter, please let us know so we can confirm your e-mail information. Many e-mail programs have spam filters. You may want to check your system and add FIT4MOM Phoenix Scottsdale to your “acceptable” e-mail list so that our e-mails are not blocked. ****Please email jillianyoung@fit4mom.com for all business matters, we cannot respond to business emails via social media platforms

CREDITS/REFUNDS

There are no credits or refunds given for missed classes due to weather or personal circumstances for ANY of our fitness programs.

HOLD POLICY

Members may place their membership on hold for reasons such as; pregnancy leave, long extended vacations, injuries, etc. All holds can be for a period of no less than 30 days and no longer than 4 months each year, not to exceed two different hold periods per calendar year. All Clients wishing to activate holds must fill out a hold form https://forms.gle/jkJzJ8UsmXd7xG5ZA. Any questions regarding holds must be sent to Jillian at jillianyoung@fit4mom.com. Once the end of the hold period has been reached, the Member’s original Membership contract will resume. Members must fill out the hold form 15 days prior to the desired hold start date. Any requests received after a Member’s billing date will be prorated accordingly- holds do not need to coincide with billing dates. There is a one time $15 charge to place a membership on hold. If a Member wishes to cancel during a hold period, the Member will still be responsible for full payment of their existing contractual agreement- hold periods DO NOT count towards the Member's monthly contractual obligation.

Month-to Month Memberships may be cancelled so long as the Member provides at least 30 days notice prior to their next scheduled billing date. ALL Membership Packages automatically renew on a monthly basis unless written notice is received by FIT4MOM Phoenix Scottsdale. In order to cancel a membership, the Member must fill out a cancel form https://forms.gle/o76yY74MADdcFcYu5. Any questions regarding cancellations must be emailed to jillianyoung@fit4mom.com No refunds will be given for dues once it has been paid. This membership is not transferable to another person or anywhere outside the FIT4MOM Phoenix Scottsdale franchise. Once a membership is cancelled, if the member wants to re-enroll they will be charged half the normal registration fee. Instructors are not authorized to cancel memberships.

STROLLER POLICY FOR STROLLER STRIDES/STROLLER BARRE/STRIDES360

Children must remain in their stroller throughout class for their safety and the overall safety of the class.

**INCLEMENT WEATHER: Although rare, FIT4MOM Phoenix Scottsdale will cancel classes in cases of inclement weather. Class cancellations due to these conditions will be announced at least 1 hour prior to class start time. Announcements of any such cancellations will always be communicated via email. Feel free to call or text us at 818-651-2424 Monday- Friday during regular business hours (9am-4pm) for any questions.

Occasionally, classes may be canceled due to scheduled events in which FIT4MOM Phoenix Scottsdale is a sponsor or has an official race team. Our offices will be closed December 23-January 2 to allow our Team to spend quality time with their families.

By purchasing this contract, you are agreeing to the terms of this contract. Terms of this agreement may change with proper advance written notice.
